My 12 year old son has a rash on his shoulders. It is in patches and is red and bumpy looking. It doesn't ooze and isn't crusty or scaly. He says it itches a little but not bad. He has been treated for scabies twice, then dry skin, and finally poison oak. Still don't have a clue what it is. Any suggestions?

When a rash has been treated four times, it still isn't clear what it is, and it still isn't better - it might be time to have a dermatologist take a look.
Scabies is unlikely if the itching was never significant, but if it were scabies the rash is an allergy to mite droppings buried in the skin. Treatment kills the mites but doesn't remove their droppings, so it can take time for fresh skin to grow out and the rash and itching to disappear.
Contact allergies can also cause shoulder rashes - poison oak is one type, but there are plenty of other things, including personal care products, he might be allergic to.
Ringworm is a patchy fungal infection that wouldn't improve with poison oak or scabies or dry skin treatment. The shoulder are not the most common spot, but possible.
